Just a simple question, were sword belts worn instead of or in addition to regular belts?

In which period?
When you look at early medieval depictions, it's very often just the sword belt which is worn.

More probably in addition to a regular belt, in that case. Both belts would be relatively narrow, as well.

(There are several older threads about suspension systems in specific periods, especially late medieval or early renaissance, full of images. Definitely worth looking for.)
